Well-led: what CQC and Ofsted inspectors actually look for
Well-led is the key question that drives overall ratings. Here is how to evidence governance, culture and learning.
Why well-led matters most
Across both CQC and Ofsted inspections, services rated poorly in leadership rarely achieve a good overall outcome. Inspectors treat governance as the engine that keeps the other domains safe.
The four questions to ask yourself
Do we know how good we are? You should have audits, KPIs and feedback that give you an honest picture. If your audits are always green, inspectors will assume they are not looking hard enough.
Do we know how we know? Evidence, not opinion. Supervision records, incident trend analysis and complaints logs should all lead to documented learning.
Do we know what to do about it? A live improvement plan with owners and dates, reviewed at least monthly.
Can staff tell the same story? Inspectors triangulate. If your team cannot describe your values, whistleblowing route or last lesson learned, the paperwork will not save you.
Practical evidence for well-led
- Minutes of governance meetings showing challenge, not just reporting.
- A duty of candour log with examples of apologies given.
- Staff survey results and what changed as a result.
- Your Statement of Purpose, reviewed within the last year.
Common pitfalls
Managers often hold everything in their heads. Write it down, date it, and show the loop closing. Use the Readiness Score on your dashboard to spot gaps before an inspector does.
